Working Alongside Your Hospice Team

Hospice services are provided by a licensed agency of your choosing, and we work closely alongside their team to support your loved one. We follow the hospice plan of care, stay attentive to changes in comfort or condition, and promptly share concerns with the hospice nurse. We also welcome the nurses and aides who visit your loved one and work together to make sure everyone is on the same page. You should never feel like you are the one carrying messages between us. We are here to communicate, coordinate, and work together so you can spend more time simply being there for your loved one.

Yes. We welcome residents who receive hospice services from a licensed hospice agency. We work alongside the hospice team to follow their plan of care and support your loved one’s changing needs.

Our caregivers are awake and present around the clock, including overnight. If we notice a significant change in your loved one’s condition, we communicate with you and the hospice team so you are kept informed and can be there when you are able.

You choose the licensed hospice agency, and their team provides the hospice services. Our caregivers continue to provide the day-to-day residential care, comfort, personal care, and companionship your loved one needs. We work together as one care team.

Absolutely. Your family remains an important part of your loved one’s care. We welcome you to spend time together, bring familiar things from home, and simply be present.

That happens, and it is a good day when it does. Your loved one can continue living with us. We reassess their care needs and continue supporting them as those needs change.

Hospice and residential care are separate services. The hospice agency provides and bills for hospice services, while the cost of living at We Care Home Living Facility LLC covers your loved one’s room, meals, personal care, and daily support.
