How We Determine Your Level of Care

At We Care Home Living Facility LLC, care rates are based on the amount of assistance your loved one requires, not on any single diagnosis or label. Every resident receives a thorough assessment before move-in to ensure we can meet their needs and provide the right level of care from day one. 

Our assessment takes into account the following:

  • Pre-admission evaluation completed by our administrator
  • Physician’s Report (LIC-602A) completed before or at admission by a licensed physician
  • A Needs and Services Plan (LIC-603) documenting daily living needs, cognition, mobility, and health conditions

Understanding Ambulatory vs Non-ambulatory

In California, a resident is considered ambulatory if they can independently leave the building in an emergency. This is not simply about whether they can walk.

  • Ambulatory: Can independently move to safety in an emergency, even if they use a wheelchair or walker
  • Non-Ambulatory: Cannot independently evacuate due to physical limitations or cognitive impairment 

Reassessment

We reassess every resident to ensure their care level always reflects their actual needs. 

Reassessment Occurs:

  • 30 days after move-in
  • Every six months thereafter
  • After any significant change such as hospitalization, a fall, new diagnosis, or cognitive decline
  • If we observe significant behavioral or cognitive changes in your loved one, we will notify the physician and or family directly and request a medical evaluation.
  • At the request of the resident, family, or physician

Our Admission Process

At We Care Home Living Facility LLC, we believe a good fit benefits everyone. We take the time to carefully get to know each prospective resident and their family to ensure we can provide the excellent care they deserve.

Take the initial steps and join us:

Step 1: Contact Us
Contact us by phone or email to learn more about our home, ask any initial questions, and schedule a visit. 

Step 2: Come Visit Us
We invite your loved one and your family to visit our home in person. This is their opportunity to meet us, see the space, ask questions, and most importantly decide if We Care feels like home to them. We want this to be their decision.

Step 3: Assessment
Our administrator will conduct a thorough assessment using the state required LIC-603 Appraisal form to determine the appropriate level of care and ensure we can provide everything your loved one requires.

Step 4: Paperwork
We will guide your family through the required state forms. Your loved one’s physician will need to complete the LIC-602A Physician’s Report and a Tuberculosis test prior to or at admission.

Step 5: Family Information
You will provide basic information such as identification, insurance, and contact details so we can prepare the admission documentation.

Step 6: Sign the Agreement
You and your loved one meets with our administrator to review and sign the admission agreement.

Step 7: Prepare the Room
Make it feel like home! You are welcome to personalize your loved one’s shared or private room with photos, decorations, and small personal belongings before move-in day.

Step 8: Move In Day
Your loved one becomes part of our family!

Step 9: Settling In Together
Our administrator works closely with your family for the first 30 days to ensure your loved one is comfortable, happy, and well cared for. We are with you every step of the way.

Our Care Levels

No two people need the same kind of care, and what someone needs today may look different a year from now. That is why we offer five levels of care, so the support your loved one receives always fits who they are and where they are. We will sit down with you, get to know your family member, and help you find the right fit. And as things change, we change right along with you.

Level 1: Independent Living Support

For those who still do most things on their own and simply want a warm, safe home with good company. We take care of all meals and snacks, housekeeping and laundry, gentle medication reminders, help arranging rides, and a friendly check in every day. Our activities are always open to everyone.
